3. HIPAA and special categories
Public website forms are not intended for protected health information or special-category health data under GDPR Article 9. Do not include patient identifiers, diagnoses, or clinical notes.

Inquiry records are retained according to regional business and regulatory needs, protected with access controls appropriate to commercial inquiry data.
